The letter of nomination accompanying this form should include and/or address the following:

• The nominee’s complete chronology of employment history with the Teaching-Family Model;
• The nominee’s contributions to the development, refinement, implementation, or dissemination of the Teaching-Family Model;
• The significance and the impact of his/her professional contributions at the community, state, national and/or international level;
• His/Her influence on the development, refinement or operation of the Teaching-Family Association;
• An up-to-date resume for the nominee; and,
• The names, addresses, and phone numbers of at least three people in the Association who could comment on the nominee’s work.

DEADLINE FOR NOMINATIONS:
Award nominations are being accepted immediately. Deadline for submission is AUGUST 1, 2001.
