Job Posting: Program Case Manager

Position Summary:

This is a work from home position three (3) days a week. Looking for an individual with knowledge of DIDD and Choices Program(s). This person must be able to articulate program values to members, family model providers, natural supports and external stakeholders. The case manager must know and adhere to Group Effort’s policies and procedures, and Teaching Family Association Standards of Service, philosophies, and goals. Have the technical knowledge and competence in the agency protocol, admission policies and transition processes.

Duties:

This candidate will be responsible case management duties involving 25-30 individual supported files. Additional duties include but not limited to:

File Management :( medical files, comprehensive files, financial files)
- Conduct MAR auditing
- One/Two Home visits per month 
-Assist with the oversight and compliance of community based services 
-Facilitate Meetings
- Act as liaison to assist members with establishing community resources 
-Can help with or conduct agency trainings 
-Participating on Agency's and third party committee's 
-Provide shift coverage if there is a staff shortage when necessary 
- Monitor Medical needs and set appointments as needed.

QUALIFICATIONS:
Bachelor’s degree with at least 1 year of experience in a related field. 
Valid Driver's License.
Able to complete online and classroom training to include

CPR and First Aid
CPI
Pass a background check
Pass a drug screen

Pay rate: $31,500.00-$33,000.00

Pay Comments: depending on experience, education & license
